Rulings: -Fix ranks before putting the Lesser Fiend into play. -The level of the target is based on the level the character had while in play, not in the discard pile. -The location can be regardless of formation. So, for example, you may kill a Bescez who is in the opposing formation and replace it with the Summon Lesser Fiend.
